As soon as you start to hold your pee, you bladder's cylindrical sphincters close up to keep urine from leaking through your urethra. These muscles are great at their job, especially if you only hold you pee once in a while.
If you hold you pee on a regular basis, you are in risk of such long-term effect such as infection, and can seriously weaken your bladder muscles which can lead to urinary retention. If you don't know what this means, it basically means that you are unable to fully empty your bladder when you pee, which makes you feel like you have to pee all the time.
